Document Submission

If you need to submit documents to your TEACH account (not transcripts)…

• Go to the following NYSED website and follow the steps given.  Please send Work Experience Verification Form to Zenita Ifill at zifill@newvisions.org.  You do not need mentoring working in charter; But if you were mentored, please send Mentorship Experience Form to Matt Gill at mgill@newvisions.org .

If your name has changed/ is different from your transcripts/exams review and complete the steps on the NYSED website for Name Changes on TEACH .

If you need to submit official transcripts you cannot submit this; your undergraduate/graduate school must send the official transcripts directly to TEACH. This website can assist you with how to submit transcripts.
